- Name of Jefferson Unitarian Church (JUC) group sponsoring the proposed project including a Letter of Sponsorship (as an appendix to the proposal).
- Name of partnering outside organization, if applicable. Append a statement of its Mission and Objectives.
- (a) Dollar amount requested with (b) Budget summary and justification – specify spending plan with primary line items.
- Who is responsible for implementation and reporting? List JUC member and member of Partnering Organization, if applicable, and give contact information.
- What are responsible person’s relevant experience and skills?
- Which one or more of the five types of social action will be addressed: Direct service, education, witness, advocacy for social change, or community organizing?
- Detailed description of proposed project, including a timeline for its implementation.
- Describe how your project will fulfill the Taguchi Social Action Fund goals for a just, compassionate and equitable society, as listed in the Guidelines above?
- What specific outcomes do you seek, hope for, expect? How will you measure your success?
- What will be the lasting value of your project?
